    • A 44-year-old woman with Crohn’s disease presents with severe abdominal pain. She was previously on infliximab but has been asymptomatic off therapy for 15 years. She is admitted to the hospital with terminal ileal Crohn’s disease and has persistent symptoms despite inpatient medical therapy. Risk factors for VTE development in this patient include:
    • A 35-year-old man undergoes an urgent ileocolic resection for medically refractory Crohn’s disease. He presents to the ER two weeks post-operatively with nausea, abdominal bloating and constipation. He has not been compliant with his extended LMWH prophylaxis due to cost. He is hemodynamically stable with normal laboratory evaluation. You recommend:
